What to do
- Enter your Index Number (continuing students) or Admission Number (freshers/applicants).
- Select your Date of Birth exactly as it appears on your official records.
- Tick the checkbox to confirm you have read the notice, then click Next.
Note: After submission, the University Clinic/Medical Doctor will conduct a medical examination to confirm
your final eligibility for the Policy Implementation Advisory: Free Tertiary Education for Persons with Disabilities (PWDs).
